  • Defining palliative care and hospice. What are the similarities and differences, as well as some of the myths associated with this type of care
  • How PF patients can access both types of care and when it is appropriate to do so in the PF journey
  • What is the importance of advance care planning
  • What is the difference between all the forms and documents, such as a living will, durable power of attorney for health care, DNR, DNI, POLST, etc
  • Ideas about how to have important conversations with family members regarding advanced directives
